COVID-19 Vaccination Policy
In the midst of the COVID-19
pandemic, Corbex Inc. wants to assure you of its continued
commitment to maintaining a safe and healthy workplace and that we
are taking additional measures to protect you, your coworkers, and
your families from contracting and spreading COVID-19. Now that
COVID-19 vaccines have been approved by the US Food and Drug
Administration (FDA) and are readily available to the general public
in Corbex Inc.’s geographic location, we are implementing a
mandatory vaccination policy. This policy is based on guidance from
the Centers for Disease Control and Prevention (CDC) and NYC
Department of Health and Mental Hygiene order issued on August 24th
2021

, and is designed to comply with all applicable federal, state, and
local laws. Compliance with this policy is a condition of your
continued employment. Please read this policy carefully.
Information About COVID-19 Vaccines
The CDC has promoted the benefits
and safety of approved COVID-19 vaccines. In addition, NYS
Department of Health and CDC recommends that all individuals who can
safely do so get vaccinated against COVID-19.
According to the CDC:
• COVID-19 vaccines currently approved or
in development in the US do not contain the COVID-19 virus and will
not make you sick with COVID-19.
• Getting the COVID-19 vaccine will not
make you test positive for COVID-19.
• COVID-19 vaccinations have been shown to
be highly effective at preventing you from getting sick with
COVID19.•
COVID-19 vaccines do not change your DNA.
• COVID-19 vaccinations are an important
tool in helping to stop the pandemic.
• COVID-19 vaccines will be free to you,
though some charges may be billed to your insurance company.

Other Infection Control Policies
This Mandatory Vaccination Policy
is a key part of our overall strategy and commitment to maintaining
a safe and healthy workplace in light of the COVID-19 pandemic. This
policy is designed for use together with, and not as a substitute
for, other COVID-19 prevention measures, including Corbex Inc's:

We need your full cooperation and
compliance with this and other health and safety workplace policies
to make them effective. Vaccination Requirements Following the CDC's
guidance and other applicable public health guidance/ requirements
of NY State Department of Health, to prevent the infection and
spread of COVID-19, and as an integral part of its public health and
safety measures,
Corbex Inc. requires that all
employees/individuals entering the workplace, including temporary
workers and independent contractors, for whom the COVID-19 vaccine
is authorized by the FDA to get vaccinated against COVID-19 and show
proof of [an FDA-approved] COVID-19 vaccination.
Employees who fail to comply with these
requirements will be barred from entering the job-site
You must schedule and receive the first
dose of your vaccine no later than September 27th2021.
You are responsible for scheduling and
obtaining all recommended doses of an FDA-approved COVID-19 vaccine
[or a COVID-19 vaccine granted Emergency Use Authorization by the
FDA].If you
already are fully vaccinated, you must provide written proof of
vaccination.
Policy
Modification
Government and public health guidelines
and restrictions and business and industry best practices regarding
COVID-19 and COVID-19 vaccines are changing rapidly as new
information becomes available, further research is conducted, and
additional vaccines are approved and distributed. Corbex Inc.
reserves the right to modify this policy at any time in its sole
discretion to adapt to changing circumstances and business needs,
consistent with its commitment to maintaining a safe and healthy
workplace.
Covid-19
Face Mask Policy
All employees, sub-contractors, consultant
personnel or visitors must use and wear required PPE (Personal
Protective Equipment) on all job-sites, including at a minimum, a
face mask, and may include gloves, a gown, and/or a face shield.
Face masks should be worn so that they
cover the individual’s nose and mouth, and the type of mask should
be consistent with CDC guidance. This means the face mask should
help prevent respiratory droplets from traveling into the air and
onto other people when the person wearing the face mask coughs,
sneezes, talks, or raises their voice. As examples, face masks that
are crocheted or beaded would not be considered sufficient for
purposes of this policy. Masks and other recommended PPE will be
obtained and supplied by Corbex Inc.
Exceptions:
There are certain situations where a face
mask is not required to be worn while on job-site and are as
follows:1.
When actively engaged in eating or drinking;
2. If an exemption in an active executive
order issued by the governor of New York State applies
If an employee who is required, declines
to wear a face mask as required, the supervisor is responsible for
addressing it with the employee. If they still refuse to comply they
will be subjected to disciplinary action which may include
employment termination.
Social Distancing
Policy
Because most respiratory droplets can
travel up to 6 feet, CDC recommends physical distancing, also called
“social distancing,” which means keeping a space of at least 6 feet
between employees. There may be certain scenarios where physical
distancing is not quite possible, and therefore a combination of
other strategies such as wearing masks, and hand hygiene should be
implemented.
Temperature Check and Health Screening
Protocols Employees will be screened for symptoms of Covid-19 at the
beginning of their shift. Employees are to self-monitor throughout
their shift and report any new or emerging signs or symptoms of the
infectious disease. Employees will be sent home if they are
experiencing any of the specified COVID-19 symptoms.
Cleaning and
Sanitation Policy
Dedicated areas, facilities, tools and
equipment that are used by one or more person should be cleaned
daily.•
Disinfect surfaces like doorknobs, tables, shared tools and
equipment, faucets, and handrails regularly.
• Alcohol based solutions that contain at
least 60% alcohol should be used to clean surfaces, tools and
equipment.•
Use proper PPE when sanitizing, such as disposable gloves. When
removing PPE, first safely remove and dispose of gloves. Immediately
clean your hands.
Health and Safety
Policy
• Sick workers are required to stay home
• Workers who develop symptoms during the
workday must immediately notify their supervisor and be sent home.
• Workers who are well but have a sick
family member at home with Covid-19 should notify their supervisor
and follow CDC recommended precautions
• Use disposable products whenever
possible•
Continuously evaluate the risks and exposures on the jobsite
